Adaptive Home Health in Detroit seeks an experienced RN Case Manager to deliver comprehensive in-home assessments and high-quality patient care. You will develop and manage the plan of care with physicians, supervising LPNs and HHAs on your cases—while representing Adaptive to patients, families, and referral sources.

This field-based, full-time role offers a salary of $85,000–$105,000 with mileage plus a $1k referral bonus and full-time benefits. Michigan RN license required.

Qualifications

• Active Michigan RN license.
• Reliable transportation and valid driver's license.
• Home health / OASIS experience strongly preferred.

• In-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care with a focus on wound care.
• Build and manage the plan of care with physicians; supervise LPNs and HHAs on your cases.
• Be the face of Adaptive to patients, families, and referral sources.

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

• Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.
• Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
• Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
• Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
