Job summary

Adaptive Home Health is expanding in Detroit and seeks an RN Case Manager for field-based, full-time work. You will deliver direct patient care, perform in-home assessments, and manage the plan of care with physicians while leveraging an AI-native platform to streamline documentation.

Qualifications

• Active Michigan RN license.
• Home health / OASIS experience preferred.
• Reliable transportation and valid driver's license.
• High ownership of caseload and charting.
• Ability to work in a field-based, full-time role.

Responsibilities

• Conduct comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC).
• Provide direct, high-acuity patient care with emphasis on wound care.
• Build and manage plan of care with physicians; supervise LPNs and HHAs.
• Represent Adaptive to patients, families, and referral sources.

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

• Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.
• Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
• Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
• Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care, with a focus on wound care
• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
