Healthcare Enterprise in New York City is seeking a Registered Nurse Care Manager to support individuals with serious mental illness, chronic conditions, and social needs by coordinating care and connecting them with resources.

You will conduct in-person and remote assessments, develop personalized care plans, and advocate across healthcare systems to improve outcomes.

Qualifications

• Active NYS RN license
• Bachelor's Degree in Nursing or equivalent
• 1–2 years of clinical experience including care management
• Strong assessment, documentation, and communication skills
