Job summary

Mitchell Martin is seeking a Registered Nurse for Behavioral Health CTI Team in Manhattan, NY. This full-time, onsite role offers a Monday–Friday schedule, with options for 09:00am–05:00pm or 08:00am–04:00pm, to support vulnerable New Yorkers during critical life transitions.

You will assess health, assist hospital-to-community transitions, provide education, coordinate care, and work with an interdisciplinary team to connect participants with essential services in a community-based setting.

Qualifications

• NYS RN license and registration required.
• Minimum 1 year of post-licensure nursing experience preferred, ideally with behavioral health populations or community/home visits background.
• Strong knowledge of health promotion, disease prevention, and community resources.
• Ability to work flexibly in field-based settings.

Responsibilities

• Assess and monitor participants' physical, cognitive, and psychosocial health.
• Support transitions from hospital/ED to community with care planning and follow-up.
• Provide health education, medication reconciliation, and disease prevention guidance.
• Link participants to community resources and healthcare providers.
• Collaborate with clinicians, care managers, and peer specialists to ensure continuity of care.
• Conduct field visits and provide flexible, community-based support.
