Pay Range: $81,000 - $90,000 Annually (Commensurate with experience)

Central Minnesota Mental Health Center (CMMHC) has a rewarding opportunity to serve as a RN within our Assertive Community Treatment (ACT) Team in St. Cloud. The RN is an active member of an evidenced-based clinical team, providing medically necessary treatment, rehabilitation, and support services to identified recipients with severe and persistent mental illness (SPMI). This position is responsible for managing the medication system; administering and documenting medication treatment in collaboration with the Psychiatrist, Nurse Practitioner (NP) or Certified Nurse Specialist (CNS).

WHAT IS ASSERTIVE COMMUNITY TREATMENT?

Assertive Community Treatment (ACT) is an intensive, non-residential mental health service for persons with serious mental illness. These individuals commonly experience multiple barriers to successful functioning, including co-occurring substance abuse or dependence, homelessness and unemployment. ACT teams help individuals treat and manage their mental illnesses and develop the skills they need for life in the community of their choice.

ACT services are:

  • Provided by multidisciplinary team
  • A blend of treatment, rehabilitation, therapeutic, and supportive services
  • Directed to persons with a serious mental illness who require intensive services
  • Offered on a time-unlimited basis and available 24 hours per day, 7 days per week, 365 days per year.
Requirements
DUTIES AND RESPONSIBILITIES

Each day you will engage with our ACT clients, helping them to have purposeful and fulfilling lives. You will get to know the clients by assisting with treatment plans that are centered around a client’s own personal strengths, needs, and aspirations for the future.

The duties may include but are not limited to:

  • In collaboration with the other nurses on the team, develop, revise maintain and ensure adherence to medication policies and procedures, including transcribing, administering, and recording medication prescribed.
  • Assignment of medication administration including transcription of medication orders; ordering medication from pharmacies; administering oral and injectable medications; and documentation medication administration.
  • Initial and ongoing evaluation, with documentation of effectiveness, client’s reactions, receptiveness, and compliance to the medications, as well as complications and side effects of psychotropic medications prescribed.
  • Provide initial and ongoing medication education to recipients, along with documentation of understanding and readiness to learn.
  • Identification of need for formal training to other ACT team members regarding psychotropic medications, symptoms, response to medications and potential side effects.
  • Participate in daily team meetings, interdisciplinary team meetings and consulting with team psychiatrist weekly regarding shared clients.
WHAT YOU’LL NEED
  • Licensed by the State of Minnesota as a Registered Nurse.
  • The Mental Health Practitioner shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17.
  • Valid Driver’s License and acceptable driving record.
  • CPR and First Aid certification.

SCHEDULE

Full-Time: Monday – Friday 7:30 AM – 4:00 PM This position would include some evening and weekend shifts and an on‑call rotation (which offers additional on‑call stipend/pay)
