Job description

Pay Range: Commensurate with experience (Behavioral Health Practitioner: Starts at $22.29; Behavioral Health Practitioner + Bachelor’s Degree or Higher: Starts at $24.69)

Schedule: Full-Time: Monday – Friday 7:30 AM – 4:00 PM with rotating on‑call evenings/weekends.

Opportunity

Central Minnesota Mental Health Center (CMMHC) offers a Behavioral Health Practitioner position within the St Cloud Assertive Community Treatment (ACT) team. The role provides a continuum of recovery‑oriented, skill‑building interventions under the supervision of a Mental Health Professional.

What Is Assertive Community Treatment?

Assertive Community Treatment (ACT) is an intensive, non‑residential mental health service for individuals with serious mental illness who often face multiple barriers to successful functioning, including co‑occurring substance abuse, homelessness, and unemployment. ACT teams help clients manage their mental illnesses and develop life skills to thrive in the community of their choice.

ACT Services Are
  • Provided by a multidisciplinary team
  • A blend of treatment, rehabilitation, therapeutic, and supportive services
  • Directed to persons with a serious mental illness who require intensive services
  • Offered on a time-unlimited basis and available 24 hours per day, 7 days per week, 365 days per year
Duties And Responsibilities
  • Implement services in the treatment plan for each recipient under clinical supervision.
  • Drive to client homes or various locations within the community where services are provided, and transport clients.
  • Establish therapeutic relationships with assigned clients.
  • Provide medication monitoring and education regarding health maintenance to encourage a healthy lifestyle.
  • Complete assessments and maintain client documentation.
  • Coordinate services with other programs and providers.
Requirements
  • Qualify as a Mental Health Practitioner (MHP).
  • Minimum of 4,000 hours working with those with mental health, developmental disabilities, or chemical dependency diagnoses. Hours may be part‑time over several years or 2 years full time.
  • Or hold a bachelor’s degree in behavioral sciences or related fields and complete a practicum, internship, or employment requiring direct interaction with adults or children served, focused on behavioral sciences (2,000 hours).
  • Or hold a master’s degree or another graduate degree in behavioral sciences or related fields.
  • Clear DHS background study.
  • Valid Driver’s License and acceptable driving record.
