Job summary

Central Minnesota Mental Health Center is seeking a Behavioral Health Practitioner housing specialist to join our ACT team in St. Cloud. You will work in the community to help clients find and sustain housing, advocating with landlords and coordinating housing supports.

You’ll conduct outreach in clients’ homes and neighborhoods, document services, and collaborate with a multidisciplinary team to promote recovery and stability for individuals with serious mental illness.

Qualifications

• Qualify as a Behavioral Health Practitioner (BHP)
• 4,000 hours working with individuals with mental health, developmental disabilities, or chemical dependency diagnoses
• Bachelor’s degree in behavioral sciences or related field (or master’s degree) with practicum/internship or employment involving direct interaction with adults or children served
• Clear DHS background study
• Valid Driver’s License and acceptable driving record
• Experience providing housing support services or related field in community settings

Responsibilities

• Help people find and maintain a place to call home by assisting with housing searches, applications, subsidies, and other housing resources.

• Collaborate with an interdisciplinary ACT team and other organizations to coordinate services.

Assertive Community Treatment (ACT) is a unique, community-based model in which a multidisciplinary team brings services directly to the people we serve. Instead of expecting clients to come to an office for support, ACT team members meet people where they are—in their homes, neighborhoods, workplaces, and communities—and work alongside them to achieve their goals.

As the Housing Specialist, you will be an essential member of this team, helping people with serious and persistent mental illness find, obtain, and maintain housing that is safe, stable, and consistent with their personal goals. You will build relationships, problem-solve alongside clients, advocate with landlords and community partners, and help clients develop the skills and supports they need to successfully live in the community.

This is not a typical office-based housing position. You will spend your time in the community, working directly with clients and collaborating closely with an interdisciplinary team. You will have the opportunity to see the impact of your work in people's everyday lives.

HOW YOU’LL MAKE AN IMPACT

As an active member of an evidence-based clinical team, you will provide medically necessary treatment, rehabilitation, and support services to identified clients with severe and persistent mental illness (SPMI).

You will:

• Help people find and maintain a place to call home by assisting with housing searches, applications, supportive housing opportunities, subsidies, and other housing resources.


• Meet clients where they are, providing services in homes, neighborhoods, community settings, and other locations that are meaningful to the client.


• Assess housing-related strengths, needs, preferences, and barriers and help clients build the skills needed to obtain and maintain independent living.


• Advocate for clients to receive the appropriate level of housing and supports while promoting independence, choice, and self-determination.


• Build relationships with landlords, property managers, housing providers, community agencies, families, and other natural supports.


• Help clients navigate challenges that can put housing at risk, including landlord concerns, lease issues, changes in income or benefits, roommate conflicts, and other barriers to maintaining tenancy.


• Support clients through transitions between housing settings and assist with relocation when needed.


• Participate in treatment planning and discharge planning for clients receiving inpatient or Crisis Residential services.


• Provide care coordination within CMMHC and with outside organizations and providers, including entities identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination.


• Maintain current knowledge of the mental health, housing, medical, financial, and social service systems and connect clients with resources that support their recovery and stability.


• Establish meaningful, trusting relationships with assigned clients and their supports and meet with each client as planned.


• Participate actively in the ACT team's daily operations, including team meetings, consultation, problem-solving, and coordination of services.


• Complete responsibilities related to housing contracts and documentation.


• Under the direction of the Mental Health Professional, develop and maintain client files according to the requirements of Minnesota Statute 256B.0623. Document all services provided accurately, completely, and within required deadlines.


• Work as part of a true multidisciplinary team. ACT team members share responsibility for clients and collaborate across disciplines to provide the services each person needs.

ACT services are designed around a team approach and provide a single point of responsibility for clients' treatment, rehabilitation, and support needs. Services are delivered in the community and are available as part of an intensive, community-based model.

Requirements

WHAT YOU’LL NEED

Qualify as a Behavioral Health Practitioner (BHP)

4,000 hours working with those with mental health, developmental disabilities, or chemical dependency diagnosis: which can be part- time over several years or 2 years full time.

Or

Holds a bachelor’s degree in behavioral sciences or related fields and completes a practicum or internship or employment that (1) requires direct interaction with adults or children served, and (2) is focused on behavioral sciences or related fields (2,000 hours).

Or

Holds a master’s degree or another graduate degree in behavioral sciences or related fields.

• Clear DHS background study.


• Valid Driver’s License and acceptable driving record.

• One or more years of experience providing housing support services or advanced education that involved field training in housing services to individuals with mental illness.


• Ability and willingness to provide services in community settings and travel throughout the service area.

• Experience navigating housing resources, supportive housing programs, subsidies, and funding sources.

• Experience working with individuals with serious and persistent mental illness.


• Experience working with landlords, property managers, housing authorities, or other community housing providers.


• Knowledge of public benefits and how changes in income or benefits can affect housing stability.


• Experience providing services in clients' homes or other community settings.


• A strong belief in recovery, self-determination, and the importance of helping people live successfully in the community.


• The ability to build genuine relationships, think creatively, and stick with a client through difficult challenges.


• Comfort with a workday that is rarely the same twice.
