Job summary

Central Minnesota Mental Health Center (CMMHC) is seeking an ACT Assistant Team Lead to join the St. Cloud team.

ACT is hands-on, community-based, team-driven care, where therapists work with a multidisciplinary team to meet ACT standards and support clients in living in the community. The role blends leadership with direct clinical work, requiring a Master’s degree and licensure toward LP/LPCC/LICSW/LMFT, with a focus on delivering high-quality, flexible services across home and community

Qualifications

• Master’s degree in psychology, social work, or marriage and family therapy.
• Current licensure as an LP, LPCC, LICSW, or LMFT or working toward licensure.
• Clear DHS background check.
• Valid driver’s license and acceptable driving record.
• CPR and First Aid certification.

Responsibilities

• Provide individual, group, and family therapy when clinically indicated.
• Provide services in clients’ homes and throughout the community.
• Adapt Individual Treatment Plans as clients’ symptoms and needs change.
• Complete Diagnostic Assessments and contribute to case formulation and treatment planning.
• Coordinate care across CMMHC programs and with outside providers.
• Assist with referrals and screenings.
• Under supervision, provide medication monitoring and health education, support medication adherence, deliver medications when appropriate.

Monday–Friday, 7:30 a.m.–3:30 p.m.

Rotating weeks of after-hours/weekend clinical coverage as part of the ACT team

ACT services are provided by a multidisciplinary team and combine treatment, rehabilitation, therapeutic, and supportive services. Services are intensive, available on a time‑unlimited basis, and designed to support individuals in living successfully in the community of their choice.

The Opportunity

As theACT Assistant Team Lead, you’ll play a key role in the clinical leadership and day‑to‑day success of the team.

Working within a shared leadership model alongside the ACT Team Lead and team psychiatrist, you’ll help ensure the team meets ACT standards as outlined in the Tool for Measurement of ACT (TMACT), while supporting high‑quality services for clients and a strong, collaborative team environment.

This is a role for someone who enjoysboth leadership and direct clinical work—someone who can step back and see the big picture while also being willing to roll up their sleeves and jump in when the team or a client needs support.

What You’ll Do

• Support compliance with ACT standards outlined in the Tool for Measurement of ACT (TMACT).
• Provide direction, clinical support, and leadership to team members to promote quality and stability in daily operations.
• Assume clinical oversight when delegated or when the Team Lead is unavailable.
• Provide individual, group, and family therapy when clinically indicated.
• Provide services in clients’ homes and throughout the community.
• Adapt Individual Treatment Plans as clients’ mental health symptoms, behaviors, and needs change.
• Complete Diagnostic Assessments and contribute to case formulation and treatment planning.
• Coordinate care across CMMHC programs and with outside providers and community partners.
• Assist with referrals and screenings.
• Under the direction of the psychiatrist and Registered Nurse, provide medication monitoring and health education, support medication adherence, deliver medications when appropriate, and communicate medication concerns to the RN.
• Participate in required team meetings and other mandatory meetings.
• Help the team problem‑solve complex situations and identify creative ways to support clients in achieving their goals.

Requirements

WHAT YOU WILL NEED

• Master’s degree in psychology, social work, or marriage and family therapy.
• Clinical Trainee working toward licensure as an LP, LPCC, LICSW, or LMFTORcurrent licensure as an LP, LPCC, LICSW, or LMFT.
• Clear DHS background study.
• Valid driver’s license and acceptable driving record.
• CPR and First Aid certification.
