Interim HealthCare Great Lakes Health Partners in Jacksonville, FL is seeking a Registered Nurse for Home Health on a day shift. This full-time role serves patients across Jacksonville, delivering skilled nursing care and coordinating closely with families and physicians.

You will assess needs, develop plans of care, educate patients, supervise aides, and document visits. Requirements include Florida RN license, 2-year degree, CPR, valid driver’s license, and EMR comfort.

Qualifications

• Florida RN license (active) required.
• Two-year nursing degree or equivalent.
• CPR certification for adults, children, and infants.
• Valid driver’s license with reliable transportation.
• Experience in home health, hospice, or home care is a plus.

Responsibilities

• Assess patient needs at start of care and at each required time point.
• Partner with patients, families, and physicians to set goals and build the plan of care.
• Deliver skilled nursing care as ordered and initiate preventive and rehabilitative nursing procedures.
• Counsel and educate patients and families to manage health with confidence.
• Direct, support, and supervise aides assigned to patients.
• Track outcomes, communicate changes, and coordinate referrals and discharge with the care team.
• Document visits clearly, accurately, and on time.

What You'll Do

What You Bring

• Graduate of an accredited school of nursing with a current, unencumbered FloridaRN license
• Home health, hospice, or home care experience a plus
• Current CPR certification for adults, children, and infants
• Comfort with electronic medical records and documenting on a laptop, tablet, or mobile device
• Valid driver's license, auto insurance, and reliable transportation
• Strong clinical judgment, communication, organization, and documentation skills
