Job summary

Interim HealthCare is seeking an experienced Registered Nurse (RN) for a Home Health role serving Jacksonville and surrounding areas. You will assess patients, plan and deliver skilled nursing care, and coordinate with families and physicians to ensure safe discharge and follow-up.

Qualifications

• Current, unencumbered Florida RN license.
• Home health or hospice experience preferred.
• CPR certification for adults, children, and infants.
• Proficient with EMR on laptop or tablet.

Responsibilities

• Assess patient needs at start of care and at defined time points.
• Deliver skilled nursing care as ordered and initiate preventive procedures.
• Counsel and educate patients and families for self-management.
• Coordinate care with physicians, families, and aides; document visits clearly.
