Oak Knoll Health and Rehabilitation LLC in Alabama seeks a skilled Registered Nurse (RN) to provide direct nursing care to residents, support rehabilitation plans, and collaborate with the clinical team to ensure the highest standards of care in our long-term care and rehabilitation facility.

The RN will administer medications, monitor health conditions, mentor nursing assistants, and educate residents and families about care plans, safety, and health conditions to promote dignity and well-being.

Qualifications

• Valid RN license

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting
• Assess, monitor, and document residents' health conditions and progress
• Administer medications and treatments as prescribed, ensuring compliance with facility protocols
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
• Provide education and support to residents and their families regarding care plans and health conditions
• Maintain accurate and timely documentation in compliance with state and federal regulations
• Foster a safe, comfortable, and therapeutic environment for residents
• Supervise and mentor nursing assistants to ensure quality care delivery
