South Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with the clinical team to ensure high standards of care in our long-term care and rehabilitation facility.

The role requires a valid RN license, strong geriatric and rehab care skills, excellent communication, and the ability to work in a team.

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting
• Assess, monitor, and document residents' health conditions and progress
• Administer medications and treatments as prescribed, ensuring compliance with facility protocols
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
• Provide education and support to residents and their families regarding care plans and health conditions
• Maintain accurate and timely documentation in compliance with state and federal regulations
• Foster a safe, comfortable, and therapeutic environment for residents
• Supervise and mentor nursing assistants to ensure quality care delivery

• Hourly Pay Range: $32.65–$42.85/hour (based on experience)
