Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in accordance with applicable laws, regulations, and agency policies.

• Coordinate the total care plan with healthcare professionals and ensure continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Conduct initial and comprehensive nursing evaluations, determine primary care focus, develop the care plan within state guidelines, and submit accurate documentation in collaboration with physician orders.
• Perform comprehensive assessments at designated times and develop appropriate care plans.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.

Education and Experience

License Requirements

• Current RN licensure in NJ
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to dependable transportation
