Job summary

Phoenix Home Care & Hospice in Springfield, MO seeks an RN for Private Duty Care and VA programs to provide in-home visits, medication setups, and routine nursing assessments. You will collaborate with physicians, supervise in-home aides when needed, and support clients and families in the comfort of their own homes.

Qualifications

• Active Registered Nurse license in Missouri is required.
• In-home care experience preferred.
• Strong organizational and communication skills.

Responsibilities

• Provide basic patient care including vital signs and medication setup per physician orders.
• Open new cases, obtain consents, and document in compliance with policy.
• Observe and report changes in a client's condition to the care team.
• May supervise in-home aides for care services.
• Educate patients and caregivers on medications and care plans.

We are currently seeking an RN for our Private Duty Care and VA departments to cover In-Home visits. This position will be responsible for aide oversight visits and routine nursing visits; including medication set ups and maintaining the plane of care. In this role, you will have the opportunity to make a difference in the lives of our clients by providing high-quality care and support in the comfort of their own home. These services are either privately paid, funded by a Long-Term Care Insurance policy, or the VA.

Responsibilities:

• Provide basic patient care: Taking Vitals, Weekly medication Management per Physicians orders, Monthly Diabetic Nails, Monthly Skin Monitoring and Bi Yearly General Health Assessments
• Opens new cases & obtains all related consents, documentation & deposits in accordance with company policy
• Observes, records & reports reaction to treatment & any changes in client's condition to appropriate personnel and/or physician
• May supervise In Home Aides for Advance Personal Care (APC) Services
• Provide instruction/ Education on Medication when needed.
• Follow state approved care plan.
• Attend quarterly Nurse Meetings
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse license in the state of MO
• In Home experience preferred
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver's license
