Job summary

BrightStar Care of Schaumburg is seeking an experienced Registered Nurse to provide in-home health care with a patient-centered approach. You will work under RN oversight, perform assessments, implement plans of care, and communicate with families to ensure quality at home.

Qualifications

• Current unrestricted RN license in state where care is provided.
• Certified CPR and negative TB skin test or chest x-ray.
• Minimum one year of documented experience in home health, primary care clinic, or health facility preferred.
• Possess exceptional nursing assessment skills and analytical thinking.
• Licensed driver with valid automobile insurance and reliable transportation.
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull; lift up to 50 pounds.

Responsibilities

• Provide thoughtful care to client/patients based on personalized Plan of Care.
• Monitor and track changes in client/patient conditions and inform Director of Nursing and relevant personnel.
• Proactively updates client/patients’ family on plan of care, health changes, and concerns.

Responsibilities
