Job summary

Complete Care Management in Maryland is hiring an RN Registered Nurse for Direct Care to support residents recently discharged from hospital and those needing long‑term care. You will deliver medications, monitor health, manage treatments and contribute to individualized care plans.

Collaboration with physicians, therapists, social workers and families is essential as you educate and advocate for residents.

Qualifications

• Licensed to work in Maryland as a RN Registered Nurse
• Able to pass background check, drug screen, professional references check & physical evaluation
• Professionally & clinically skilled with leadership qualities

Responsibilities

• Administer medications (oral, topical, IV, injectable)
• Perform wound care/prevention and manage pain
• Conduct health assessments, monitor changes, and analyze data
• Create, update and execute individualized care plans
• Collaborate with physicians, therapists, social workers and others
• Educate residents and families about health status, changes and care expectations
• Maintain detailed documentation of health status, care provided and progress
• Oversee and delegate to LPNs and CNAs to ensure quality care and teamwork

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Assessment & Monitoring: Conduct health assessments, track changes & analyze data

Care Planning: Create, update & execute individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate to LPN's and CNA's ensuring quality care/teamwork
