Brighten Life Home Health in Utah is seeking a Registered Nurse for Home Health in Davis/Weber County. The role is PRN with a pay range of $36.00–$43.00 per hour, including responsibilities such as admissions, daily nursing care, and care coordination.

The nurse will assess patients, develop and update the plan of care, administer medications, and support families while coordinating with physicians. Ideal candidates are licensed RNs in good standing in Utah, with home health experience preferred

Qualifications

• Must be a Registered Nurse in good standing with the State of Utah.
• Home Health Experience Preferred.
• Physically able to lift, turn, position and transfer patients.
• Reliable transportation with current auto insurance and driver's license.
• Demonstrates knowledge of infection prevention and control.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and documents findings.
• Regularly re-evaluates patient nursing needs and revises the plan of care as needed.
• Initiates and revises the plan of care based on patient status and nursing diagnoses.
• Administers medications and treatments as prescribed and counsels patient/family.
• Provides education to patient and family and coordinates discharge planning as needed.
• Participates in on-call duties and ensures necessary items and services are arranged.

Responsibilities to include: admissions, catheter changes, medication needs, falls, urgent needs that need to be addressed and documenting all care performed in accordance with the plan of care.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs. Including a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
• Participates in on-call duties as defined by the on-call policy.
• Ensures that arrangements for equipment and other necessary items and services.
• Instructs, supervises and evaluates home health aide care provided every two (2) weeks
• Prepares clinical notes and updates the primary physician when needed and every 60 days.
• Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
• Communicates with community health related persons to coordinate the care plan.

Qualifications

• Must be a Registered Nurse in good standing with the State of Utah. Maintains a current license throughout employment.
• Home Health Experience Preferred.
• Physically able to lift, turn, position and transfer patients.
• Have reliable means of transportation, current auto insurance and driver’s license.
• Demonstrates working knowledge of infection control principles and practices.
