Job summary

Jobtailor in New Jersey seeks a Registered Nurse to oversee care plans and supervise LPNs in a group home setting. The role includes direct medical care, check-ins with individuals, and coordination with external providers to ensure comprehensive services.

Responsibilities

• Oversee care delivery and monitor medical needs in the group home
• Conduct regular check-ins with individuals to assess wellbeing
• Provide medication education and assist with administration and documentation
• Coordinate with external medical providers to ensure seamless care
• Maintain accurate medical, dental, and related records and participate in IDT meetings

• Create and manage individualized plans of care
• Create care plans for LPNs to follow
• Conduct check-ins with individuals at the group home
• Provide medical care and monitor and follow through on care
• Provide medication education and assist with medication administration and documentation
• Encourage normalization and independence of individuals served
• Train and assist individuals served and staff on medical education and related areas
• Liaise with outside medical providers to coordinate care
• Maintain accurate written medical, dental, and related records
• Intervene in emergencies involving individuals served
• Attend interdisciplinary team (IDT) meetings
• Oversee Licensed Practical Nurses (LPNs)

Requirements

• Currently Licensed Registered Nurse (RN) in NJ
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
• Ability to build rapport with patients
• Experience working with individuals with developmental disabilities preferred
• Availability Mondays and Tuesdays, 7am–7pm
