Job summary

• Conduct check-ins with individuals at the group home.
• Provide necessary medical care and monitor and follow through on care.
• Provide medication education and related support.
• Encourage normalization and independence of individuals served.
• Provide training and assistance to individuals served and staff on medical education and related areas.
• Liaise with outside medical providers to coordinate care.
• Assist with medication administration and required documentation.
• Maintain accurate written records regarding medical, dental, and related matters.
• Intervene in emergencies when needed.
• Attend interdisciplinary team (IDT) meetings.
• Review medication logs, doctors’ appointments, follow-ups, and staff training.

Requirements

• Currently Licensed Registered Nurse (RN) in NJ
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
• Ability to build rapport with patients
• Experience working with individuals with developmental disabilities preferred
