Job summary

Nlc-Support in New Jersey is seeking a Registered Nurse to join our team. You will create and manage care plans for individual patients, oversee LPNs, and ensure patient-centered care in our group home setting.

You should have current NJ RN licensure, strong problem-solving skills, and the ability to observe and communicate effectively. Flexible work-from-home options are available, with part-time shifts on Mondays and Tuesdays from 7am to 7pm.

Qualifications

• Currently Licensed Registered Nurse (RN) in NJ.
• Excellent interpersonal and client care skills.
• Valid driver’s license.
• First aid/CPR certification required.
• Ability to build rapport with patients.

Responsibilities

• Create plans for LPNs to follow and check in with residents.
• Provide medical care, monitor care, and educate on medications.
• Encourage independence and normalization of individuals served.
• Train individuals and staff on medical education and related areas.
• Liaise with outside medical providers to coordinate care.
• Assist in administration of medications and documentation.
• Maintain accurate medical and related records.
• Intervene in emergencies and attend IDT meetings.
• Experience with developmental disabilities preferred.

Job Summary

We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You will oversee Licensed Practice Nurses (LPN). You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

Responsibilities

• You will create plans for LPN's to follow to care for the individuals as well as do check ins with the individuals at the group home.
• Provide necessary medical care, monitoring of care, follow through of care, medication education and other related areas.
• Encourage the normalization and independence of individuals served.
• Provide training and assistance to individuals served and staff on medial education and other related areas.
• Act as a liaison to outside medical providers to ensure coordination of care for the individuals served.
• Assist in the administration of medications and required documentation for the individuals served.
• Maintain accurate written records in connection with medical, dental, and other related matters for the individuals served.
• Intervene when needed in emergencies with regard to the individuals served.
• Attend interdisciplinary team (IDT) meetings
• Experience working with individuals with developmental disabilities preferred
