Job summary

Brighton/Suncrest Hospice is seeking an empathetic Registered Nurse to provide compassionate hospice care in patients' homes and in facilities across our service area. You will collaborate with an interdisciplinary team to develop and implement patient-centered care plans that respect comfort, dignity, and family preferences.

This role emphasizes thoughtful time management, accurate EMR documentation, and the ability to travel throughout the service area.

Qualifications

• Current RN license in good standing in the state of practice.
• A passion for providing compassionate hospice care.
• Strong collaboration and communication with interdisciplinary teams.
• Ability to travel throughout the service area and manage time effectively.
• Proficiency with EMR documentation and scheduling.

Responsibilities

• Provide compassionate, individualized hospice care in patients' homes and facilities.
• Collaborate with nurses, social workers, chaplains, CNAs, and volunteers to develop care plans.
• Maintain lower clinician caseloads and focus on meaningful patient time.
• Document care accurately in the EMR and coordinate with the care team.
• Support families with education and in-the-moment decision making.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves. Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice
• A passion for providing exceptional, patient-centered hospice care
• Strong collaboration and communication skills with interdisciplinary care teams
• Ability to work independently while effectively managing your daily schedule
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs
• Comfort learning and utilizing electronic medical record (EMR) systems
• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area
