Suncrest Hospice is seeking an RN to provide compassionate, in-home and facility-based care across a service area. You will join an interdisciplinary team, delivering clinical care while addressing psychosocial needs and coordinating community resources.
The role supports case management, admissions, or resource functions, with travel across the service area as a core aspect of the job.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
