Brighton/Suncrest Hospice is seeking a Registered Nurse to provide direct patient care during home visits, SNF, and assisted living settings. You will join an interdisciplinary team to address psych-social needs and coordinate resources across the service area.
As an RN, you will contribute in Case Management, Admissions, or Resource roles, ensuring high quality care and adherence to hospice regulations. Travel is a part of the role with reliable transportation required.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
