Suncrest Hospice is seeking a Registered Nurse to provide patient-centered care in homes and facilities within the service area. You will be part of an interdisciplinary team, offering psych-social assessments and coordinating community resources.
Roles include Case Management, Admissions, or Resource coordination. You will travel across the service area with reliable transportation, maintaining documentation and complying with hospice regulations.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
