Job summary

PurposeCare and its affiliate companies provide home health services to communities across the Midwest, offering compassionate care and flexible scheduling for clinicians. This Home Health Registered Nurse role supports patients in their own homes with autonomous decision-making and a focus on holistic care.

The position operates on a paid per-visit basis with rates ranging from $40 to $105 per visit. PurposeCare emphasizes growth opportunities, comprehensive benefits, and a supportive team

Qualifications

• Current license with the state Board of Nursing.
• 1 year of nursing experience is required - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
• Current Driver's License, vehicle insurance, and/or access to dependable transportation.

Responsibilities

• Provide skilled home health visits to patients in their homes.
• Assess patient needs and develop care plans in collaboration with families.
• Coordinate with caregivers and other healthcare professionals to ensure seamless care.
• Maintain accurate and timely documentation of visits and patient progress.

Hiring Range
