Job summary

PurposeCare is seeking a Home Health Licensed Practical Nurse (LPN) to join our team across multiple locations in Indiana, including South Bend. The role offers flexible scheduling, autonomy in patient care, and the opportunity to deliver holistic care to our homebound clients.

Qualifications

• Current license with the state Board of Nursing (or equivalent).
• 1 year of nursing experience required; home health and OASIS experience preferred.
• Ability to perform physical tasks (up to 50 lbs), bending, standing.
• Valid Driver's License and transportation access.
• Background check required.

Responsibilities

• Provide skilled visits in patients' homes addressing their needs.
• Deliver high-quality home health care with autonomy and compassionate care.
• Coordinate care with the care team to maintain patient safety and comfort.

Qualifications and Requirements:

• Current license with the state Board of Nursing
• 1 year of nursing experience is required - home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Hiring Range

This is a paid per visit position - Rates range from $30 - $35 base
