Qualifications

Responsibilities

• Coordinates resident care with Administrator, Director of Nursing and other disciplines to ensure quality care.
• Engages and partners to develop care plans and provide continuity of care.
• Administers scheduled medication to residents in a timely manner and supports quality assurance activities.
• Answer call lights and communicate needs to relevant staff, prioritizing timely responses.
• Demonstrates strong clinical assessment and supervisory skills.
• Knowledge of CMS/state regulations and SNF standards.
