Job description

Summary

This Registered Nurse - Remote Patient Monitoring - Home Telehealth Care Coordinator position is located in the Telehealth service at the West Haven campus. It is a full-time position, 40 hours per week.

Qualifications

Basic Requirements:

  1. English Language Proficiency
  2. Graduation from an accredited school of professional nursing (ACEN or CCNE)
  3. OR enrollment in a master's level nursing bridge program with coursework equivalent to a bachelor's degree
  4. OR foreign nursing graduates with current unrestricted registration
  5. Current, full, active, and unrestricted registration as a professional nurse in the US
  6. Graduate Nurse Technician (GNT) exception for those pursuing licensure
  7. Grade determinations based on education, experience, and credentials, detailed in the provided online assessment link
  8. Preferred experience in Telehealth outpatient services
  9. Physical ability to perform duties; pre-placement physical exam required
Duties
  • Perform responsibilities demonstrating leadership and participation in HT experiences
  • Manage complex patient care creatively and cost-effectively
  • Apply care and case management principles using health informatics and HT technologies
  • Improve clinical outcomes, access to care, and reduce complications and hospital visits
  • Conduct initial and ongoing patient assessments, including reviews of systems
  • Triages and assesses data from HT patients
  • Identify and intervene in potential exacerbations and coordinate timely care
  • Communicate with community providers and interdisciplinary teams
  • Support patient self-management through education and counseling
  • Facilitate communication regarding patient progress and clinical trends
  • Ensure understanding of care plans and question practices for evidence-based care
