Position Details

• Location: Telecommuter position, but must be able to travel to Farmington, CT for training/meetings
• Department: Case Management
• Schedule: Full time, 40 hours/week, Monday through Friday, 8:00AM – 4:30PM

If you have a CT nursing license and are based in CT, MA, NY, or RI, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities

• Member Care Coordination: Collaborate with physicians and multidisciplinary teams to coordinate and communicate care plans for members.
• Member Referral Support: Assist providers, members, and families in obtaining referrals and provide support and counseling as appropriate.
• Care Plan Development: Develop and maintain member care plans with designated physicians.
• Clinical Improvement: Participate in activities to improve the clinical experience for members and physicians.
• Liaison: Communicate with care team members to coordinate needs.
• Provider/Member Education: Educate on community and health plan benefits.
• Additional duties as needed in conjunction with Medical Management.

Required Qualifications

• Bachelor’s Degree in Nursing (BSN)
• Current CT Nursing License from CT, MA, NY, or RI
• 2+ years managing complex disease states
• 2+ years of telephonic health plan case management
• Experience with Electronic Health Records (Allscripts, Epic)
• Knowledge of NCQA/ACO/CDO regulations
• Proficiency with multiple computer programs (Microsoft, Adobe)
• Ability to travel to Farmington, CT for training and meetings; possible travel elsewhere

Preferred Qualifications

• Master’s Degree in Nursing (MSN)
• Certified Case Manager Certification (CCMC)
• 5+ years as a nurse case manager
• Experience in managing complex disease states and utilization management
• Experience working remotely
• Bilingual skills in English and Spanish, Polish, Mandarin, or Vietnamese

Salary range: $59,500 to $116,600 annually, based on factors like experience, education, and location. Benefits include comprehensive packages, incentives, stock options, and 401k.
