Job summary

Mayo Clinic is seeking a Registered Nurse for our Primary Care Community Internal Medicine Outpatient practice in Minnesota. The RN will coordinate nursing care, provide direct patient care, and engage in patient/family education and transitions of care.

The role involves collaboration with providers, care team members, telehealth, and web-based care to support patients across the continuum. The RN will support professional nursing practice, lead through preceptor and quality improvement

Qualifications

• Bachelor's degree in nursing from an accredited program; RN license.
• 1 year RN experience in applicable care setting or LPN experience at Mayo Clinic for certain tracks.
• Excellent verbal and written communication skills; ability to work in a team.

Responsibilities

• Coordinate nursing care including direct patient care and patient/family education.
• Assist with transitions of care and telehealth/web-based care as part of Primary Care Community Internal Medicine Outpatient practice.
• Provide leadership, mentoring, and participate in quality improvement efforts.
