Job summary

Aptive Resources seeks a registered nurse to joining its healthcare staffing team. You will perform patient check-in, disease management, and coordination of care across inpatient and outpatient settings, including IV therapy and telemetry monitoring.

Collaboration with the interdisciplinary team and accurate EMR documentation are essential to ensure quality patient care. Responsibilities include coordinating admissions, discharges, and transfers, while extending access through secure messaging

Qualifications

• Current, unrestricted RN license from any state in the United States.
• Current Basic Life Support (BLS) certificate accredited by the American Heart Association (AHA).
• Minimum of four (4) years of current nursing experience including at least two (2) years inpatient care coordination and two (2) years in outpatient settings.
• Education must have been completed at a school accredited by ACEN or CCNE.

Responsibilities

• Provide patient check-in and disease management.
• Scrub appointments and coordinate with the interdisciplinary team.
• Increase patient access through MyHealthyVet/secure message responses.
• Assist with admissions, discharges, transfers, and delegations to assistive personnel.
• Document in electronic medical records and participate in IV therapy and telemetry monitoring.
