Job summary

HealthPartners is seeking a Registered Nurse to provide high-quality, patient-centered care. The role emphasizes comprehensive nursing assessments, development of care plans, and delegation of nursing tasks within a collaborative multidisciplinary team.

The RN will contribute to education plans for patients and families, ensuring continuity of care across settings. The position requires MN RN licensure, Wisconsin RN licensure within two months of hire, and a current BLS certification.

Qualifications

• Graduate of an accredited school of nursing
• Current RN licensure in Minnesota
• Current RN licensure in Wisconsin within two months of hire
• Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable
• Strongly demonstrated written and oral communication skills

Responsibilities

• Provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition.
• Delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity
• Creates and implements dynamic teaching plans based upon identified patient need
• Acts as a resource for other staff
• Care Coordination: uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans
• Documentation: documents accurately and completely and keeps record regarding clinical practice
• Communication Skills: demonstrates positive verbal and written communication and effective listening with internal and external customers
