Job summary

Hackensack Meridian Health is seeking a Registered Nurse to provide direct patient care across a specialized population. The role focuses on delivering evidence-based nursing, coordinating care with a multidisciplinary team, and supporting patient and family education.

Key responsibilities include comprehensive assessments, developing care plans, safe medication administration, and coordinating discharge planning to ensure seamless transitions and high-quality outcomes for diverse patients.

Qualifications

• RN license required (New Jersey)
• BLS/CPR certification by American Heart Association required
• BSN in progress and/or willing to acquire within three years of hire
• Six or more months of nursing experience preferred

Responsibilities

• Assess, plan, implement and evaluate care for patients.
• Perform age- and disease-specific assessments and data collection.
• Develop and coordinate a patient/family-centered care plan.
• Communicate plans with nursing peers, physicians and support services.
• Administer medications using bar-scanning and documentation technologies.
• Coordinate multi-disciplinary discharge to ensure safe transitioning of care.
