Qualifications

Responsibilities

• Delivers safe and appropriate nursing care to patients per the nursing skills inventory and patient age/needs.
• Coordinates nursing care planning with an interdisciplinary team and makes clinical decisions for patient care.
• Collaborates with healthcare team; develops discharge and transfer plans as needed; ensures adherence to standards of care.
• Participates in program evaluations and process improvements to achieve client-centered outcomes.
• Promotes compassionate, personalized care and highest service standards while adhering to regulations and Memorial Hermann policies.
