Job summary

Heartland Hospice seeks an On-Call Registered Nurse to provide compassionate hospice care in patients' homes and facilities. The RN triages after-hours calls, makes urgent visits, assesses symptoms, and implements care plans focused on comfort and dignity.

The nurse collaborates with an interdisciplinary team, communicates status changes to providers, and supports emotional and spiritual needs, honoring patient goals and cultural values while maintaining regulatory and quality standards.

Qualifications

• Must be a licensed RN in good standing.
• Hospice or palliative care experience preferred.
• Ability to work independently delivering care at patients' homes and facilities.
• Excellent communication and compassionate caregiving skills.

Responsibilities

• Triage after-hours calls and determine urgency.
• Make urgent home or facility visits when needed.
• Assess and manage patient symptoms and adjust care plans.
• Administer medications and monitor response.
• Educate patients and families on care goals and comfort options.
• Document care activities promptly and accurately.
• Collaborate with the interdisciplinary team and communicate changes to providers.
• Support emotional and spiritual needs while honoring patient goals and cultural values.
