Job summary

Heartland Hospice seeks an On-Call Registered Nurse to provide compassionate hospice care to patients in homes and facilities. The RN triages after-hours calls, makes urgent visits, assesses symptoms, and implements care plans focused on comfort and dignity.

You will collaborate with an interdisciplinary team, educate patients and families, maintain timely documentation, and honor patient goals and cultural values while adhering to regulatory and quality standards.

Responsibilities

• Triage after-hours calls and coordinate urgent visits.
• Assess symptoms and implement care plans focused on comfort and dignity.
• Manage pain and symptom control and administer medications as prescribed.
• Provide patient and family education on hospice care and care plans.
• Document care activities in a timely and accurate manner.
• Collaborate with an interdisciplinary team and communicate status changes to providers.
• Support emotional and spiritual needs, honoring patient goals and cultural values while maintaining regulatory and quality standards.
