Job summary

Heartland Hospice seeks an On-Call Registered Nurse to provide compassionate hospice care to patients with life-limiting illnesses in homes and facilities.

The RN triages after-hours calls, makes urgent visits, assesses symptoms, and implements care plans focused on comfort and dignity. Responsibilities include pain and symptom management, medication administration, patient and family education, and timely documentation.

Qualifications

• Licensed RN in Missouri or applicable state with hospice practice experience.
• Experience delivering end-of-life care with focus on comfort and dignity.
• Ability to make rapid clinical decisions in after-hours scenarios.

Responsibilities

• Triage after-hours calls and triage urgency of visits.
• Perform urgent in-home or facility visits with symptom assessment.
• Administer medications and implement care plans.
• Educate patients and families on care goals, pain and symptom management.
• Document care activities timely and accurately.
• Collaborate with interdisciplinary team and update providers on status changes.
• Support emotional and spiritual needs, honoring patient goals and cultural values.
• Adhere to regulatory and quality standards.
