Job summary

Qualifications

• Active RN license in good standing.
• Hospice or palliative care experience preferred.
• Excellent communication and patient education skills.

Responsibilities

• Triage after-hours calls and determine urgent care needs.
• Make urgent home and facility visits to assess patient condition.
• Implement and monitor individualized care plans emphasizing comfort and dignity.
• Administer medications and manage symptoms per physician orders.
