Job summary

Essentia Health is seeking an experienced RN MSHO Care Coordinator to manage care for dual-eligible Minnesota Health Care Program members. You will assess needs, coordinate services, educate patients, and connect them with resources through an interdisciplinary team.

This role involves remote work with required home visits within the Essentia footprint (Duluth area and nearby). Eligible candidates must have MN RN license, MnCHOICES, BLS, and driver’s license, with strong care coordination

Qualifications

• Graduate of 2- or 4-year Registered Nursing Program.
• 2+ years of experience in care coordination or case/care management.
• Current Registered Nurse License in the State of Minnesota.
• MnCHOICES Assessor certification (or ability to obtain within 30 days of hire).
• Current Basic Life Support certification (or ability to obtain within 90 days of hire).
• Must have a valid driver’s license.

The Care Coordinator conducts assessments, coordinates home and community-based services and assists members in making progress towards his or her health goals while coordinating care with the interdisciplinary team.
