Essentia Health seeks a skilled home health nurse to assess, plan, implement, evaluate and coordinate patient care across medical, diagnostic and emergency needs. You will work within a team to deliver high-quality home care and ensure adherence to professional standards and Essentia Health policies.

Responsibilities include direct patient care, care coordination, and ongoing assessment of patient conditions.

Qualifications

• Current nursing licensure in the state(s) of employment.
• BCLS certification or ability to obtain within 1 month from date of hire.
• Minimum of 1 year of clinical nursing experience.
