Job summary

The role requires a BSN or nursing degree, RN license, and experience with Medicare wellness programs; strong patient education and documentation skills are essential. Full-time, on-site with standard business hours.

Qualifications

• RN licensure and nursing degree as prerequisites
• Experience with Medicare wellness visits or preventive care
• Proficiency in comprehensive patient assessment and documentation

Responsibilities

• Lead RN-led Medicare Wellness Visits and complete preventive care assessments
• Document health history, medications, functional status, and social determinants
• Perform screens (depression, cognitive, fall risk, functional, preventive health)
• Identify and close preventive care gaps (immunizations, screenings)
• Provide health education, coaching, and self-management support
• Develop personalized prevention plans and follow-up care
• Escalate abnormal findings per protocol
• Coordinate referrals and follow-ups with care teams
• Maintain accurate EHR documentation adhering to CMS/regulatory requirements
• Participate in quality improvement and program performance initiatives
