Advocate Health seeks a Registered Nurse to lead Medicare Wellness Visits in outpatient settings, performing preventive care assessments and health coaching. You will document patient health histories, screenings, and individualized wellness plans while coordinating referrals and follow-ups with the care team.

You will ensure accurate EHR documentation, meet Medicare requirements, and contribute to quality improvement initiatives that enhance patient outcomes and population health.

Formación

• Graduate from an accredited School of Nursing. BSN preferred.
• Current RN license and BLS required.
• 2 years of clinical experience preferred; specialty certification within 2 years if not currently certified.

Responsabilidades

• Conduct RN-led Medicare Wellness Visits with preventive care assessments and screenings.
• Document comprehensive patient assessments including health history and functional status.
• Perform required screenings such as depression, cognitive, fall risk, and preventive health assessments.
• Identify and close preventive care gaps including immunizations and cancer screenings.
• Provide patient education, health coaching, motivational interviewing, and self-management support.
• Develop personalized prevention plans and follow-up care needs.
• Coordinate referrals, tests, and appointments with providers and team members.
• Maintain accurate EHR documentation in compliance with policies.
• Participate in quality improvement and workflow optimization initiatives.
