Job summary

Cooper Norcross Health is seeking a Wellness Coordinator RN to perform Medicare Wellness visits for CCA patients and collaborate with the care team to ensure high-quality, comprehensive care. This PRN on-site role is scheduled 3 days per week (Tue/Wed/Thu, 8–5).

Responsibilities include pre-visit planning, care coordination, and thorough documentation in the EHR to support accurate billing and patient outcomes. Strong communication and teamwork are essential.

Qualifications

• Bachelor's degree in nursing or related field.
• BSN preferred with active RN license in New Jersey.
• Experience delivering Medicare wellness visits and care coordination.

Responsibilities

• Provide annual Medicare wellness visits for CCA patients.
• Coordinate pre-visit planning and referrals to care management services.
• Document in the EHR and ensure timely billing.
• Collaborate with providers to deliver high-quality, patient-centered care.
