Job summary

Cooper Norcross University Hospital is seeking a Wellness Coordinator RN to provide Medicare Wellness visits for CCA patients and coordinate with the health care team to optimize preventive care. The role is PRN, 3 days per week, Tuesday through Thursday, 8-5, onsite.

The ideal candidate will have an active RN license, experience with Medicare wellness visits, and strong patient education and collaboration skills to support continuous improvement in health outcomes.

Qualifications

• RN with active license
• Medicare wellness program experience preferred
• Strong communication and patient education skills

Responsibilities

• Provides annual Medicare wellness visits for CCA patients.
• Assists in pre-visit planning and identifies patients who may benefit from integrated care/specialist referrals.
• Collaborates with clinicians to ensure high-quality care and timely documentation in the EHR.
• Promotes patient education and coordinates care management activities with the health care team.
• Maintains accurate and timely documentation to support billing and regulatory compliance.
