Job summary

PRECISION HEALTHCARE in Laconia, NH is hiring a Licensed Practical Nurse for a Long-Term Care/Skilled Nursing Facility. You will provide comprehensive nursing care to residents, assess and document health status, administer medications, and help develop, implement, and evaluate individualized care plans to ensure safety, comfort and well‑being.

This 13‑week contract requires active NH LPN licensure, 2 years LTC/SNF experience, EMR documentation proficiency, and the ability to float as needed.

Qualifications

• Minimum 2 years of Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience.
• Active, unencumbered New Hampshire LPN license.
• Previous travel nursing experience preferred.
• Experience caring for geriatric and medically complex residents.
• Electronic Medical Record (EMR) documentation experience required.
• Charge Nurse experience highly preferred.
• Must be able to float as needed.

Responsibilities

• Provide comprehensive nursing care to residents in a long-term care or skilled nursing facility.
• Assess, monitor, and document residents' health status and changes in condition.
• Administer medications and treatments in accordance with physician orders and facility protocols.
• Develop, implement, and evaluate individualized resident care plans.
• Monitor chronic conditions and promote residents' health, safety, comfort, and overall well-being.
• Collaborate with physicians, nursing assistants, therapists, residents, and families to ensure quality care.
• Maintain accurate and timely documentation in the facility's electronic medical record system.
• Follow infection prevention protocols, medication safety standards, and applicable state and federal regulations.
• Recognize changes in residents' conditions and promptly communicate concerns to the appropriate healthcare providers.
• Float as needed to support resident care and facility operations.

Rate: Up to $1,450.00 Gross Weekly + Benefits Available from Day 1

Shifts Available: 8-Hour Evenings

Contract: 13-Week Contract | Local & Travel Accepted
