Requirements

• Minimum 2 years of Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience
• Active, unencumbered New Hampshire LPN license
• Previous travel nursing experience preferred
• Experience caring for geriatric and medically complex residents
• Electronic Medical Record (EMR) documentation experience required
• Charge Nurse experience highly preferred
• Must be able to float as needed

Certifications

• AHA BLS required
• Additional certifications as required by the facility

Responsibilities

• Provide comprehensive nursing care to residents in a long-term care or skilled nursing facility
• Assess, monitor, and document residents' health status and changes in condition
• Administer medications and treatments in accordance with physician orders and facility protocols
• Develop, implement, and evaluate individualized resident care plans
• Monitor chronic conditions and promote residents' health, safety, comfort, and overall well-being
• Collaborate with physicians, nursing assistants, therapists, residents, and families to ensure quality care
• Maintain accurate and timely documentation in the facility's electronic medical record system
• Follow infection prevention protocols, medication safety standards, and applicable state and federal regulations
• Recognize changes in residents' conditions and promptly communicate concerns to the appropriate healthcare providers
• Float as needed to support resident care and facility operations
