Ascension Health is seeking an experienced RN Case Manager at Ascension St. John\'s Medical Center in Tulsa, OK to lead patient care from admission to discharge, developing discharge plans to reduce 30-day readmissions and ensure seamless continuity of care.

You will mentor junior case managers, review performance data to drive improvements, and collaborate with a multidisciplinary team to optimize workflows in Case Management.

Qualifications

• Licensed RN in Oklahoma
• ACMA/CCMC Case Manager credentials preferred
• Education: high school diploma/GED with 2+ years exp or higher degree; Master's preferred

Location: Tulsa, OK

Department: Case Management

Schedule: Full-Time, Monday-Friday, 8am-4:30pm

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degreerequired. Master's degree preferred.

Additional Preferences

• 5+ years of RN experience
• 3+ years of Case Management experience
• Hospital discharge planning experience preferred
