Job summary

Ascension is seeking an RN Navigator in the ICU at Ascension St. John Medical Center in Tulsa, OK. This role focuses on central communication, care coordination, and ensuring clear understanding of diagnoses and treatment plans for patients and families.

You will support transitions from ICU to step-down or home, remove barriers to timely care, provide follow-up education, and deliver psychosocial support to families facing critical illness. A valid RN license is required.

Qualifications

• Active, valid RN license required.
• Diploma or equivalent nursing education with licensure at time of hire.

Responsibilities

• Serve as central point of contact to ensure patients and families understand diagnoses and treatment plans.
• Facilitate seamless transitions from ICU to step-down units or home, ensuring continuity of care.
• Identify and address barriers to timely access to services (financial, transportation, social).
• Provide follow-up education and psychosocial support to patients and families.
• Support continuity of care and reduce readmissions through coordinated care.

How you’ll make an impact in this role

• Enhanced Communication: RN Patient Navigators serve as a central point of contact, ensuring that patients and families clearly understand complex diagnoses and treatment plans.
• Care Coordination: They facilitate seamless transitions from the ICU to step-down units or to home, reducing treatment delays and ensuring continuity of care.
• Barrier Removal: By identifying and addressing obstacles such as financial, transportation, or social issues, they ensure timely access to necessary services.
• Reduced Readmissions: Providing follow-up support and education on self-management helps decrease emergency room visits and urgent readmissions.
• Emotional Support: They provide psychosocial support to patients and their families, helping them navigate the distress associated with critical illness.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Licensed Registered Nurse obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Oklahoma Board of Nursing required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional preferences we're seeking

• 5+ years of Critical Care nursing experience, preferably in an ICU setting
