Pathways Healthcare in Worcester, MA seeks a Registered Nurse - Hospice to join a physician- and nurse practitioner-led home health and hospice team. You will assess patients, develop and revise plans of care, administer medications, and collaborate with physicians to ensure high-quality palliative care at home.

Ideal candidates have 2+ years of nursing, prefer geriatrics, and hold an accredited nursing degree with EMR proficiency. A valid driver’s license and reliable vehicle are required.

Formación

  • 2+ years nursing experience, geriatrics preferred.
  • Degree in Nursing from an accredited institution.
  • Proficient with electronic medical records.
  • Current BLS CPR certification.
  • Valid driver’s license and own reliable vehicle.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.

Responsabilidades

  • Assesses and evaluates patient’s status by writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develops a care plan based on nursing diagnosis and palliative actions.
  • Initiates preventive and rehabilitative nursing procedures and administers meds.
  • Completes and documents clinical notes and tracks pain/symptom changes.
  • Communicates with physician regarding patient needs and orders.
  • Provides and maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.

Job Description:
  • Role: Registered Nurse - Hospice
  • Service Area: Worcester territory.
  • Schedule: Monday – Friday (8:30-5:00pm)
  • Job Type: Full-time
Qualifications:
  • 2+ years' nursing experience, preferably in geriatrics is desirable.
  • New nurses, feel free to apply! We have a great training program.
  • Degree in Nursing from an accredited institution.
  • Proficient with electronic medical records.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Possess a valid driver's license and own a reliable, insured vehicle.
Responsibilities:
  • Assesses and evaluates patient’s status by:
  • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Provides and maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.
Additional Duties:
  • Participates in on-call duties.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities.
