Zusammenfassung

Pathways Healthcare is seeking a Registered Nurse - Home Health to provide patient-centered care in the Concord, MA area. The role involves assessing patients, planning and implementing care plans, and coordinating with physicians to ensure optimal outcomes.

The position is full-time, Monday through Friday, 8:30 am to 5:00 pm, with a salary range of $72,000 to $92,000 per year. Experience in geriatrics and proficiency with EMR are preferred.

Qualifikationen

  • 2+ years nursing experience, geriatrics preferred.
  • Degree in Nursing from an accredited institution.
  • Proficient with electronic medical records.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Valid driver's license and own reliable vehicle.

Aufgaben

  • Assess and evaluate patient's status and write/initiate plan of care.
  • Regularly re-evaluate patient and family/caregiver needs.
  • Participate in revising the plan of care as necessary.
  • Develop a care plan based on nursing diagnosis with family involvement.
  • Administer medications and treatments as prescribed.
  • Complete and record accurate clinical notes.
  • Communicate with the physician regarding patient needs and changes.
  • Provide and maintain a safe patient environment.
  • Attend interdisciplinary group meetings.

Job Description
  • Role: Registered Nurse - Home Health
  • Salary: $72,000-92,000/year
  • Main Office location: Concord, MA
  • Service Area: Devens, Shirley, Ayer, Pepperell, Townsend, Fitchburg, Leominster and Lunenburg areas.
  • Schedule: Monday-Friday (8:30-5:00pm)

Responsibilities
Patient Care
  • Assesses and evaluates patient's status by:
  • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
  • Provides and maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.
Additional Duties
  • Participates in on-call duties.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities
